Herbs

Horny Goat Weed: What the Studies Actually Show

Quick answer

Human evidence for horny goat weed and sex is thin. LiverTox says its claimed benefits are not proven in rigorous controlled trials, and Memorial Sloan Kettering says more studies are needed. Its best human data are for bone loss in women after menopause. Vapofil prints 8 mg, while typical products hold 250 or 500 mg.

The plant

What is horny goat weed, and what is icariin?

Horny goat weed is the common name for Epimedium, a leafy plant used in traditional Chinese medicine. Icariin is the flavonoid most often linked to it.

LiverTox, the NIH reference on liver injury, lists icariin, icaritin and epimedin as its main active flavonoids (LiverTox: Horny goat weed). The plant holds more than 200 known compounds in all.

Vapofil's label names Epimedium sagittatum, one species of the genus. It prints 8 mg per 2-capsule serving, "From 10:1 extract". No icariin percentage is given, so the amount of icariin in the capsules is unknown.

It is also the ingredient missing from the brand's six-ingredient graphic, though the label prints it. See the horny goat weed ingredient page for the short profile.

In people

What human evidence exists for horny goat weed?

Very little for sex, and somewhat more for bones. Three reference sources agree that the sexual-health claims rest on tradition rather than solid trials.

What three reference sources say about horny goat weed in humans
SourceSexual functionOther usesDose guidance
Memorial Sloan Kettering (MSK: Epimedium)Traditional use; "more studies are needed"Best data: bone loss after menopause (a 24-month trial and a 2022 meta-analysis)Not published
LiverTox (NIH) (LiverTox: Horny goat weed)"Not proven in rigorous controlled trials"Blood pressure and heart claims also unprovenSold as 250 or 500 mg, usually once a day
RxList (RxList: Horny goat weed)Insufficient evidenceInsufficient evidence for all usesNo appropriate dose range known

The bone research was done in women, not men. It says nothing about erections or desire, and nothing about an 8 mg dose.

Reading the evidence

What does "not proven in rigorous controlled trials" mean?

It means no large, well-run study has shown that horny goat weed works for sex in people. A rigorous trial assigns people at random to the herb or a dummy pill, and neither side knows which one they got.

Without that design, hope and chance can look like a result. Men who expect a change often report one.

  • Random assignment keeps the two groups alike.
  • A placebo group shows how much change happens anyway.
  • Blinding stops expectations from shaping answers.
  • Enough people lets real effects stand out from noise.

Lab or animal findings on icariin, however striking, do not fill that gap.

A case report

Can horny goat weed affect heart rhythm?

There are reports that it can. Memorial Sloan Kettering describes a 66-year-old man with heart failure who was hospitalized with a new irregular heartbeat and hypomania after two weeks of daily use (MSK: Epimedium).

A single case cannot prove the herb caused it. Still, MSK lists rapid or irregular heartbeat among the side effects, along with sweating and feeling hot at high doses (MSK: Epimedium). RxList also reports arrhythmia, and warns that large amounts may cause spasms and severe breathing problems (RxList: Horny goat weed).

Heart disease? Ask first

MSK advises people with heart disease to avoid horny goat weed. If you have a heart condition or take heart medicine, talk to your doctor before any product that contains it. A racing heart, chest pain or fainting needs emergency care.

Medicines

Which medicines interact with horny goat weed?

Blood pressure drugs, blood thinners, estrogens and heart-rhythm drugs top the list. RxList rates these as moderate interactions (RxList: Horny goat weed), and MSK adds two more (MSK: Epimedium).

Blood pressure

Pressure may drop too low

Paired with blood pressure medicine, the herb may push pressure lower than intended (RxList: Horny goat weed).

Clotting

More bleeding risk

It may slow clotting, so it adds risk with anticoagulants and antiplatelet drugs. RxList advises stopping before surgery (RxList: Horny goat weed).

Hormones

Estrogen-like effects

It may worsen estrogen-sensitive conditions. MSK says icariin may blunt aromatase inhibitors used in some cancers (MSK: Epimedium).

RxList also flags drugs that prolong the QT interval, a heart-rhythm measure (RxList: Horny goat weed). MSK notes the herb may speed how the liver enzyme CYP3A4 clears some medicines (MSK: Epimedium). Cautions for all seven Vapofil ingredients are gathered on the side effects page.

How does 8 mg compare with typical horny goat weed products?

It is a small fraction. According to LiverTox, the herb usually comes as 250 or 500 mg tablets or capsules, one a day (LiverTox: Horny goat weed). Vapofil's 8 mg is about 1/31 of the smaller size and 1/62 of the larger one (our arithmetic).

Even the 80 mg "equivalent" figure is under a third of the smaller product. That figure is the weight of herb used to make the extract, not what the capsules hold; the extract ratio post explains why.

Each "Label" bar is drawn to scale against the research dose or daily guideline beside it. Every amount is printed per 2-capsule serving; boron is worked out from the label's 5%. The label's "equivalent to" figures for tongkat ali and horny goat weed describe the dry herb used to make the extract, not what the capsules hold. Studies tested each ingredient alone, not Vapofil.

Vapofil

What do the horny goat weed studies mean for Vapofil?

They do not support a sexual benefit at any dose, and Vapofil uses far less than usual. Vapofil itself has never been tested in a published trial.

The small dose may mean less exposure to the herb's side effects. It does not remove the interaction cautions, because no safe threshold has been set. RxList goes further: the science is too thin, it says, to name any proper dose range (RxList: Horny goat weed).

The bottle itself makes no sex claim; the front says only "Premium Formula". You can read every line of the label on our Supplement Facts page.

Bottom line

Is horny goat weed worth taking for erections?

The evidence does not show that it works. Its best human data concern bone loss in women, and its heart and drug cautions are real. Erection problems deserve a doctor's visit, because they can signal heart disease, diabetes or high blood pressure.

FAQ

Horny goat weed: common questions

What is icariin in horny goat weed?

Icariin is a flavonoid, one of the main active compounds in Epimedium plants, along with icaritin and epimedin, per LiverTox. The plant contains more than 200 compounds. Vapofil's label gives no icariin percentage, so the icariin in its 8 mg is unknown.

Is there human research on horny goat weed for sexual function?

Very little of good quality. LiverTox says its claimed sexual benefits are not proven in rigorous controlled trials, and Memorial Sloan Kettering says more studies are needed. Its strongest human evidence concerns bone loss in women after menopause, not men's sexual health.

Do the horny goat weed bone studies apply to men?

Not directly. Memorial Sloan Kettering says the strongest human evidence comes from women after menopause, including a 24-month trial on bone loss. Those results say nothing about men's bones, erections or desire, and they used far more than the 8 mg in Vapofil.

Which medicines should not be mixed with horny goat weed?

Ask a doctor first if you take blood pressure medicine, blood thinners, estrogens or drugs that affect heart rhythm. RxList rates these as moderate interactions. MSK adds that icariin may blunt aromatase inhibitors and that the herb may change how some drugs are cleared.

Is horny goat weed the same plant in every product?

Not always. Horny goat weed is a common name for several Epimedium species, and LiverTox notes the plants hold more than 200 compounds. Vapofil names Epimedium sagittatum. Without a stated icariin level, two products with the same name can differ a lot.

Is 8 mg of horny goat weed enough to have an effect?

No study shows an effect at 8 mg, and no study shows a sexual effect at larger doses either. Vapofil's 8 mg is about 1/31 of a 250 mg product. Its 80 mg equivalent figure describes the herb used to make the extract, not what the capsules hold.
