Tongkat Ali in Vapofil: 10 mg Against the 100-200 mg Trial Doses

Quick answer

The Vapofil panel gives tongkat ali its own line: 10 mg of extract per 2-capsule serving. That is a tenth of the 100 mg dose, and a twentieth of the 200 mg dose, used in a 2021 trial of 105 men with low testosterone, as summarized by LiverTox (LiverTox: Tongkat ali). The "equivalent to 1000 mg" line describes dry root used to make the extract, not what you swallow.

The basics

What is tongkat ali?

Tongkat ali is a tall shrub from the forests of Southeast Asia. Its scientific name is Eurycoma longifolia, and the root is the part that is used.

In Malaysia and Indonesia the root is a folk tonic for men. Modern products sell it for energy and desire, usually as a water extract.

The Vapofil label prints it sixth of seven lines, at 10 mg per 2-capsule serving.

LiverTox, the NIH liver injury database, gives a traditional range of 100 to 400 mg a day (LiverTox: Tongkat ali).

The printed amount

How much tongkat ali is in Vapofil?

Two capsules give you 10 mg of tongkat ali extract. One capsule holds about 5 mg, and a bottle of 60 capsules holds about 300 mg in all.

The figure has its own line, outside any blend, so you can compare it with research.

Reading the label

How should you read the 100:1 ratio on the tongkat ali line?

Roughly 100 parts of dried root were concentrated into each single part of extract. By that ratio, 10 mg of extract came from roughly 1,000 mg of root. You still swallow 10 mg of extract, not a gram of anything.

The label reads: "From 100:1 extract, equivalent to approximately 1000mg of dry Tongkat Ali powder." That explanation is our reading.

Why the ratio cannot be compared with trial doses

A ratio describes how much raw root was used, not what the extract contains. Two 100:1 extracts made by different methods can hold very different amounts of the active compounds. The 2021 trial used a standardized extract measured by dose in milligrams, not by a root-equivalent figure (LiverTox: Tongkat ali).

Extract ratios are the maker's claim. No independent lab has published results for Vapofil. Read our longer piece on tongkat ali doses and extract ratios.

The evidence

What did the 2021 tongkat ali trial find at 100 and 200 mg?

It found small rises in total testosterone in older men who started with low levels. LiverTox summarizes the 2021 trial by Chinnappan and colleagues: 105 men aged 50 to 70, testosterone under 300 ng/dL, treated for 12 weeks (LiverTox: Tongkat ali).

Higher dose200 mgUp 12.5%No change

Free testosterone is the unbound share; SHBG is its carrier protein. LiverTox notes no clear athletic benefit.

WebMD agrees: any rises "appear to be small," and effects on erections and sex drive are unclear (WebMD: Eurycoma longifolia).

Dose check

How does 10 mg compare with what studies used?

It is 10% of the lower trial dose and 5% of the higher one. Matching 100 mg would take 20 capsules a day; the label says two, and you should never take more.

Each "Label" bar is drawn to scale against the research dose or daily guideline beside it. Every amount is printed per 2-capsule serving; boron is worked out from the label's 5%. The label's "equivalent to" figures for tongkat ali and horny goat weed describe the dry herb used to make the extract, not what the capsules hold. Studies tested each ingredient alone, not Vapofil.

No trial has tested 10 mg.

Safety

Is tongkat ali safe?

Side effects in short studies were uncommon: nausea, diarrhea, headache and rash, per LiverTox (LiverTox: Tongkat ali). Long-term safety is poorly documented. LiverTox rates its liver risk as "D" based on one case, a 47-year-old bodybuilder with severe liver injury.

Product quality is the larger worry. A Department of Defense program, Operation Supplement Safety, warns that some tongkat ali products tested positive for heavy metals such as lead and mercury. Others were spiked with sildenafil, the drug in Viagra (OPSS: Tongkat ali).

Hidden drugs and nitrates

Sildenafil that is not on a label can push blood pressure dangerously low when taken with nitrate heart medicines. There is no evidence that Vapofil contains any drug, but no lab test of it is published either. Read more in our post on hidden drugs in sex supplements.

The European safety note

A 2021 opinion from the European Food Safety Authority found that tongkat ali's safety "has not been established under any condition of use." Its worry was possible harm to DNA. LiverTox and OPSS both cite it (LiverTox: Tongkat ali) (OPSS: Tongkat ali).

Medicines

What should you check before taking tongkat ali with medicine?

WebMD lists propranolol, testosterone and two prostate cancer drugs as possible interactions (WebMD: Eurycoma longifolia).

Tongkat ali interactions and cautions named by WebMD (WebMD: Eurycoma longifolia). Ask a pharmacist about anything else you take.
Medicine or conditionWhy it matters
PropranololA beta-blocker for blood pressure and heart rhythm; listed by WebMD as an interaction
Testosterone therapyTongkat ali is studied for testosterone, so the two may add together
Darolutamide or enzalutamideProstate cancer drugs that block male hormones; also on the WebMD list
Cancer history or prostate problemsHormone-related conditions; talk to your provider first
Heart rhythm, kidney or liver problemsNamed by WebMD as reasons to ask before use

Vapofil also holds horny goat weed, which carries its own heart rhythm and blood pressure cautions. All seven ingredients' cautions are collected under Vapofil side effects.

Our take

Is the tongkat ali in Vapofil worth it?

In its favor

  • The amount is printed, so you can compare it with research
  • The best-studied herb on the label for testosterone
  • Side effects in short trials were uncommon

Against it

  • 10 mg is a tenth of the lowest dose tested
  • The "1000 mg" wording sounds larger than the dose
  • Long-term safety is not established

Vapofil is a dietary supplement, not a testosterone treatment, and its bottle makes no testosterone claim. The Urology Care Foundation diagnoses low testosterone only when two blood draws before noon both fall below 300 ng/dL and symptoms are present (Urology Care: Low testosterone). A doctor can order those tests.

FAQ

Tongkat ali in Vapofil: common questions

How many mg of tongkat ali are in each Vapofil capsule?

Each capsule holds about 5 mg of tongkat ali extract, since the label prints 10 mg per 2-capsule serving. A full 60-capsule bottle holds about 300 mg. The 2021 trial gave men 100 or 200 mg a day.

Can such a small tongkat ali dose have any effect?

No study has tested 10 mg, so nobody can say it does anything. The lowest dose in the 2021 trial was 100 mg a day, ten times more, and that dose produced a 9% rise in total testosterone in men who started low.

Does the 1000 mg on the label mean I get 1,000 mg of tongkat ali?

No. You get 10 mg of extract per serving. The 1000 mg figure describes the dry root that was used to make that extract at a 100:1 ratio. Nobody can verify that ratio from the bottle.

Can tongkat ali contain heavy metals or hidden drugs?

Some products have. Lead, mercury and undeclared sildenafil have all turned up in tongkat ali products, per Operation Supplement Safety. Vapofil has no published third-party test, so its purity is unconfirmed.

Should men with prostate or liver problems avoid tongkat ali?

Men with a history of cancer, prostate problems, heart rhythm problems, or kidney or liver disease should talk to a provider first, per WebMD. So should anyone on testosterone, propranolol or prostate cancer drugs such as enzalutamide or darolutamide.

Why does Europe question tongkat ali safety?

European regulators have not accepted tongkat ali as safe. A 2021 European Food Safety Authority review raised a possible DNA-damage concern and concluded that safety was not established for any use. LiverTox and Operation Supplement Safety both report this.
